Downloading Information captured through a payment form

I have created a custom form to accept payment. How can download the information captured when the form was making a payment.

I am assuming you created a payment button on paynow. The payment details wll be sent to your registered email address.

Its a form as follows:

I would like to be able to download the additional fields captured in a csv sheet.

Is that possible?
